7-OH (7-hydroxymitragynine): interactions

Naloxone reverses 7-OH-associated respiratory depression in animal studies and human case reports [1]. As with opioids generally, naloxone has virtually no effect on someone who hasn't taken an opioid [2]. As with opioids generally, naloxone precipitates withdrawal in people who are physically dependent [3].

7-OH is associated with opioid-like effects including respiratory depression [1], and in animal studies it produced opioid-like respiratory depression that was fully reversed by naloxone [1]. As with opioids generally, people commonly combine them with alcohol or other depressants such as benzodiazepines and GABA-ergics like pregabalin, and that combination is reported to make respiratory depression worse [4]. It is far more potent at the mu-opioid receptor than mitragynine and other classical opioids such as morphine [5][6][7], which is a reason for caution, not reassurance, when stacking depressants on top of it.

Beyond breathing, there is no reliable human data on how 7-OH interacts with serotonergic drugs, stimulants, or anything that changes how the body processes it. That gap is not reassurance — it means the interaction hasn't been studied, not that it's been ruled out. Treat any combination as unstudied: don't combine, don't use alone, and if you do combine anyway, start far lower than feels sensible with each substance involved.
