Methadone is prescribed almost everywhere as the racemate, but the opioid activity is nearly all in one half: levomethadone, the (R)-enantiomer, is the μ-agonist. (S)-methadone contributes little μ activity, though it is the enantiomer more associated with the QT-interval effect that makes methadone interact badly with other QT-prolonging drugs. Levomethadone is dispensed as a single enantiomer in some countries, notably Germany.
