In the United States, MDMA is a Schedule I controlled substance, defined as having no accepted medical use [1][2]. It is also reported to be classified as having a high potential for abuse [3]. No prescription can be written for it [3]. In the United Kingdom, it is a Class A drug and sits in Schedule 1 of the Misuse of Drugs Regulations 2001, with no recognised medicinal use [4]. The Netherlands controls it under Schedule I of the Opium Act; a state commission there has recommended keeping it in Schedule I specifically because of criminal-market concerns, separate from any judgment about its health effects [5].

Canada controls MDMA under Schedule I of the Controlled Drugs and Substances Act. Possession can carry up to seven years in prison; trafficking or production can carry up to life [6]. A time-limited federal exemption in British Columbia, running January 2023 to January 2026, decriminalises possession of up to 2.5 g for personal use — a narrow, local, and expiring carve-out, not a change to the drug's status elsewhere in the country [6].

Australia is a partial exception: the raw active ingredient remains Schedule 9 (prohibited), and domestic manufacture is not permitted under Commonwealth law [7], but since July 2023 MDMA preparations have been reclassified to Schedule 8 specifically for PTSD treatment under authorised psychiatrists, with a therapeutic goods standard now in place for that medical use only [7][8].
