Seizure watch 2025: xylazine at #6, medetomidine and BTMPS in the top 25 for the first time, and fentanyl's share down from its 2022 peak

Methamphetamine and cocaine remain the top-identified substances in 2025 NFLIS reports, but xylazine, medetomidine, and dozens of first-time entries mark a fast-moving edge underneath them.

1. Methamphetamine — 323,404 reports (28.7%)
2. Cocaine — 195,317 reports (17.3%)
3. Cannabis — 143,689 reports (12.8%)
4. Fentanyl — 132,210 reports (11.7%)
5. Heroin — 27,286 reports (2.4%)
